MySymptoms is a symptom tracking app that is available for Android and iOS phones[1] [2]. The app is free for almost all features but some require a one-time purchase of around $5. The app allows for data exports to CSV format and does not provide detailed analytics. It can be used to take notes and activity time slots can have notes underneath them allowing user to record custom metrics, e.g. what position they slept in.

The interface for time tracking is not as good as it could be. Also has a symptoms survey which is quick and easy to use but does not automatically notify user to fill it out.
